 Start Date/Issue number not showing
Solution The installation tried to place these 2 files, which are necessary to show and capture the extra info fields:

/components/com_virtuemart/themes/default/templates/checkout/list_payment_methods.tpl.php
/administrator/components/com_virtuemart/classes/ps_checkout.php

If you are using the default VirtueMart templates, upload the above 2 files manually.

If you are not using the default VirtueMart templates, please upload the first file (list_payment_methods.tpl.php) to the corresponding subfolder of your 'themes' folder instead (and also upload the 2nd file)
